You don’t need to fill out these forms before your office visit.
Gastroenterology of the Rockies uses an electronic registration system – patients will receive a text and/or email prior to their appointment with a link to confirm and fill out the information for your office visit. These forms are provided to you as a courtesy for review in advance of your visit:
